NONIMMIGRANT VISA APPLICATION FORMS

As is the case worldwide, the U.S. Embassy in Jakarta and the U.S. Consulate General in Surabaya require all applicants for non-immigrant visas to use the Electronic Visa Application Form (EVAF). The EVAF has been initiated worldwide by the Department of State to speed up the application process for nonimmigrant visas while increasing the accuracy in visa processing.

The EVAF may be accessed by any computer with internet access and is in the English language. All answers must be in English. However, translation help of the application form can be found at http://jakarta.usembassy.gov/consular/DS156.pdf. The EVAF website does not allow users to complete part of the application form and save the rest for later. Users must fill in all the information at one time, save it, and then print the form. 

There is no charge to fill out the form; however, Internet cafes may charge a printing and internet usage fee. Application information is not transmitted to the Embassy and is not a simultaneous request for an interview. All EVAF forms must be printed out and brought to the U.S. Embassy on the date of the interview. 

Applicants without the EVAF form will be turned away and asked to come back the following business day. EVAF applications without the barcode or those incorrectly filled out will not be accepted.

How to use the EVAF

Applicants can enter all their data on an Internet web site that produces a version of the application form with a bar code. This bar code allows Embassy employees to upload the information quickly and easily into the Embassy computer systems. Applicants with properly completed electronic visa application forms will go to the front of the check-in line and will spend less time waiting to be called up for their interview.

Computer Requirements: To use the electronic visa application forms available below, your Internet browser must support 128-bit encryption and you should use either an ink-jet or laser printer to print the completed application form. If you are using Internet Explorer (Windows), the minimum version that will work with this site is version 5.0, with service pack 2. If you are using Netscape, the minimum version that will work with this site is version 6.2. You must also have Adobe Acrobat Reader installed on your PC in order to download and print the completed application form. The minimum version that will work with this site is version 5.

Note: Depending on the speed of your Internet connection, it may take several minutes to download the initial and completed copies of the form. Please be patient. Instructions for completing an electronic visa application form DS-156:

Enter the information requested into the appropriate spaces on the form. Please answer all questions. Your answers must be in English. Your application may be rejected at the Embassy if each question does not have a complete answer.

1. Review the information you entered for accuracy.
2. Press the Continue button at the end of the form. Failure to click on the Continue button will result in an incorrectly formatted application form.
3. PRINT THE FORM AFTER IT IS RETURNED TO YOU AND DISPLAYED IN ADOBE ACROBAT
4. The barcode that prints on the last page contains the information you entered and will help us process your application more quickly. Note that there are 3 pages to the EVAF, and the applicant must bring all three pages to the interview. 
5. Bring all printed pages with you when you come in for your interview. Please do not fold the printed application form.

Please note that some applicants may need to complete other forms in addition to the DS-156. These forms are available on the Internet as interactive .pdf files. This format allows applicants to enter all their data on the screen and then print out the form to bring to the visa interview. Please note that these applications do not use bar codes.
